Grooming is one of the most normal things a cat does — until it isn't. If your cat is licking one patch thin or bald, the fur is usually trying to tell you something, and it's medical more often than it's "just stress".
Cats groom for hours a day — the skill is spotting when normal grooming has tipped into a health sign.
The Quick Answer
Cats groom a lot, and that's normal. Estimates vary by study, age, and coat, but a healthy cat commonly spends somewhere between a sixth and half of its waking hours grooming — for many cats, a couple of hours a day.1 So "my cat grooms constantly" is usually nothing to worry about.
Over-grooming is different. It's when licking, chewing, or hair-pulling starts removing fur faster than it grows back — leaving thin patches, stubble, or bald skin, often on the belly, inner thighs, flanks, or a single spot the cat keeps coming back to.
Here's the part most owners get backwards: when vets actually investigate over-grooming, they find a medical cause far more often than a purely behavioural one. In one well-known study of 21 cats referred for "psychogenic" (stress-related) hair loss, 16 turned out to have an underlying medical problem and only 2 were purely behavioural.23 So the honest first question isn't "what's stressing my cat?" — it's "what's making my cat itchy or sore?"
| Normal grooming | Over-grooming | |
|---|---|---|
| Coat | Even, glossy, no gaps | Thin patches, stubble, or bald skin |
| Skin underneath | Clean and intact | May be red, scabbed, or broken |
| Pattern | All over, unhurried | Fixated on one area — belly, inner legs, a single spot |
| Your gut feeling | Cat looks well-kept | Cat looks "moth-eaten" or keeps at one spot |
What Normal Grooming Actually Does
Grooming isn't just about being clean. For a cat it does several jobs at once:
- Temperature control — saliva evaporating off the coat helps a cat cool down.
- Coat maintenance — spreads natural oils, keeps fur waterproof and insulating, and removes loose hair and debris.
- Parasite and wound care — licking dislodges parasites and cleans minor wounds.
- Self-soothing — grooming genuinely calms cats, which is also why stress can push it too far.
Bonded cats also groom each other (allogrooming) around the head and neck — a social behaviour, not a health problem. A normally-groomed cat has an even, glossy coat with no bald spots, and the skin underneath looks clean and unbroken.
Signs It Has Tipped Into Over-Grooming
Look for the fur and the skin, not just the licking:
- Thinning or bald patches, often roughly symmetrical, on the belly, inner thighs, flanks, or back legs — sometimes called "fur mowing" or barbering.
- Short, broken, stubbly hairs where the coat is thin. This is a useful tell: the cat is snapping the hair shaft off, not the fur falling out from the root.
- Fixated licking or chewing of one area — you catch them at it repeatedly, or hear near-constant licking.
- Red skin, scabs, sores, or a rash under the thinned fur.
- More hairballs or vomiting hair than usual.
- A patch that keeps reappearing each time it starts to regrow.
Broken stubble on skin that's otherwise producing hair is a strong sign the loss is self-inflicted — the cat is grooming it off, rather than the hair dropping out because of a hormonal or internal problem. Your vet will often part the fur and look at whether the hair tips are broken or tapered.
What Actually Causes It — Medical Reasons First
Most over-grooming is driven by something that itches or hurts. The common culprits:
| Cause | What's going on | Clues |
|---|---|---|
| Fleas & flea-allergy dermatitis | The single most common cause. In flea-allergic cats, one or two bites can trigger intense, prolonged itch. | Over-grooming the lower back, tail base, and belly — even if you never see a flea.2 |
| Other allergies | Food or environmental (pollen, dust) allergy — atopic dermatitis. | Itchy skin around the face, neck, belly; may be year-round or seasonal.23 |
| Other parasites | Mites (Demodex, Notoedres), lice, Cheyletiella. | Patchy hair loss, scaling, sometimes very itchy.3 |
| Skin infection | Bacterial or yeast overgrowth, or ringworm (a fungal infection). | Redness, crusting, sometimes a spreading patch.23 |
| Pain | Cats lick over what hurts. Bladder inflammation (feline idiopathic cystitis) classically shows up as over-grooming the belly and genital area; joint pain can drive licking over a limb. | Belly-focused grooming, or licking one joint; may come with litter-tray changes.4 |
| Hormonal / internal | Conditions such as an overactive thyroid, more often in older cats. | Usually other signs too — weight loss, appetite or thirst changes. |
| Stress (psychogenic) | Anxiety or compulsive over-grooming — real, but a diagnosis of exclusion. | Diagnosed only after the above are ruled out.35 |
Don't Blame Stress First
Stress-driven over-grooming — psychogenic alopecia — is a real condition, but it's genuinely over-diagnosed.5 Vets treat it as a diagnosis of exclusion: they reach it only after ruling out fleas, allergies, parasites, infection, and pain.
That Waisglass study is exactly why. Of the cats referred as "psychogenic", 76% actually had a medical cause of itch or pain, only about 10% were purely behavioural, and the rest were a mix of both.23 Labelling a flea-allergic or sore cat as "stressed" leaves it itching or hurting while the owner buys calming diffusers that can't fix the real problem.
None of this means stress is irrelevant. It can be a real trigger or amplifier — especially with bladder inflammation, where stress and pain feed each other.4 But stress is the last box to tick, not the first.
When to See a Vet
Over-grooming is a "book a check" sign rather than a "wait and watch it get worse" one. See your vet if you notice:
- Bald or thinning patches, broken or reddened skin, scabs, or sores
- Constant licking or chewing of one area
- A rise in hairballs or hair-vomiting alongside the coat changes
- Weight loss, appetite or thirst changes, or a cat that seems generally off
Same-day / urgent: a cat — especially a male cat — straining in the litter tray, crying, passing little or no urine, and licking the genital area can have a urinary blockage, which is life-threatening within hours. That's an emergency, not simple over-grooming — see what to do when a cat can't pee and call a vet straight away.
What You Can Do at Home (Alongside the Vet)
- Treat every pet in the house for fleas, year-round — including indoor-only cats. Given fleas are the top cause, effective flea control is the single highest-value step, and it's often what finally settles the licking.
- Don't punish or startle your cat for licking. It doesn't touch the underlying itch or pain, and the added stress can make things worse.
- Lower the household stress load: a predictable routine, one more litter tray than you have cats, vertical space and hiding spots, daily play, and separate food/water/resting spots in multi-cat homes.
- Write down the pattern for your vet: where the fur is going, when it started, anything that changed at home, and whether your cat is otherwise well. It speeds up the diagnosis.
- Skip the DIY fixes: don't shave the area, don't apply human creams, and don't rely on a cone as the "solution" — it hides the behaviour without treating the cause.
The Calm Way to Think About It
Over-grooming sits in the same awkward middle as a lot of cat care: it's not usually an emergency, but it's not nothing either. The useful sequence is:
- Notice the pattern — which patch, how long, what else is going on.
- Rule out the itchy and painful causes first (fleas top of the list).
- Only then consider stress — and treat it as a real factor, not the whole story.
- Get a vet check, because "self-inflicted hair loss" almost always has a fixable reason behind it.
